Job Title: Clinical Nurse CoordinatorVNA Health Group is seeking a skilled Clinical Nurse Coordinator to join our team. As a key member of our home health care team, you will be responsible for reviewing and ensuring the accuracy of patient assessments, plans of care, and medication profiles.Responsibilities:

• Collaborate with clinicians to ensure accurate documentation of patient information
• Review and verify OASIS assessments, plans of care, and medication profiles
• Develop and maintain effective working relationships with healthcare team members
• Ensure compliance with VNA Health Group policies and proceduresRequirements:
  • RN with Bachelor's degree in Nursing
  • OASIS Certification required
  • 3-5 years of home care experience
  • Strong knowledge of OASIS and Medicare/Medicaid guidelines
