Job Description Job Description Fellowship Hospice Team is
seeking a compassionate and skilled Nurse Practitioner to join our
interdisciplinary team on a per-diem basis. This role involves
conducting face-to-face visits with patients in their homes or
residential facilities, delivering high-quality, person-centered
care. The ideal candidate will have prior hospice experience and
demonstrate a commitment to excellence in clinical practice. Travel
within Monmouth and Ocean County is required. This is an as-needed
position with flexible scheduling. However, candidates must be
available during daytime hours on at least one to two weekdays per
week to ensure continuity of care and timely patient support. Pay
Rate: $100 per visit plus mileage reimbursement. Essential
Functions: Perform comprehensive assessments of hospice patients,
including physical, emotional, and spiritual needs. Develop,
implement, and update individualized care plans in collaboration
with the care team. Conduct face-to-face visits with patients in
their homes and at facilities. Manage symptoms and provide
palliative interventions in accordance with the patient’s goals and
wishes. Educate patients, families, and caregivers about the
hospice philosophy and care process. Communicate effectively with
the care team, including physicians, nurses, social workers, and
chaplains. Ensure compliance with all regulatory and organizational
standards. Requirements: Current Nurse Practitioner license in NJ.
Certification in Hospice and Palliative Nursing (preferred but not
required). At least 1 year of experience in hospice, palliative
care, or related fields. Willingness to travel within the community
and to facilities for patient visits. Exceptional communication and
interpersonal skills. Ability to work independently while being
part of a supportive team. Work Location: On the road
