Diem Established in 1904, Moorestown Visiting Nurse Association is
a non-profit home care community health organization serving
Burlington, Camden, and Gloucester counties in New Jersey. We are
one of the very few home care agencies qualified to provide a
continuum of care for patients of all ages, no matter how serious
the problem, no matter what stage of the illness. It is care weve

currently seeking a Per Diem Social Worker to provide social work
services to our patients within a multidisciplinary team approach.
We are looking for someone who is capable of being understanding
toward our patients with unique needs. Responsibilities: Help
patients and families understand, accept, and follow medical
recommendations. Provide services that are planned to restore the
patient to optimum psychosocial and physical health. Provide
patients with appropriate community information about community
resources as needed. Participate in appropriate long-term planning
in anticipation of patient discharge from Agency services. Complete
clinical and progress notes for each contact accurately and timely.
Maintains compliance with all state and regulatory bodies along
with Moorestown VNA policies and standard operating procedures.
Qualifications: Master's Degree from school of social work
accredited by the Council of Social Work Education. Current NJ
State Social Work License. One year of work as a Medical Social
Worker in a health care setting. Home health experience preferred
