Carnegie Post Acute Skilled Nursing Facility, known for our commitment to providing high-level care to seniors post-hospitalization and post-surgery, is currently seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. This role is vital to ensuring the health and well-being of our residents through exceptional clinical practices, patient management, and healthcare administration under the supervision of registered nurses and physicians. Monitor patient health and document their medical history and condition Provide basic patient care and treatments, such as dressing wounds, administering medication, and inserting catheters Ensure the comfort and safety of patients, adhering to all facility, local, state, and federal guidelines and procedures Communicate with registered nurses and physicians, reporting critical information about patient status Assist with patient mobility and physical therapies Educate patients and their families on treatment plans and health maintenance Strong practical nursing skills in a post-acute care setting Excellent patient management and observation skills Ability to perform essential nursing tasks (e.g., taking blood pressure, wound care, patient hygiene) Effective communication and interpersonal skills Proficient in nursing software and patient documentation systems Medical Insurance Dental Insurance Vision Insurance Valid Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of practice CPR/BLS certification
